The mediastinal surface is the medial surface of each lung, facing the mediastinum. The hilum divides it into two parts:
  1. Anterior mediastinal part
    • Lies in front of the hilum.
    • Faces the heart and pericardium.
    • Shows the cardiac impression.
    • The left lung has a deeper cardiac impression because of the heart’s leftward position.
  2. Posterior vertebral part
    • Lies behind the hilum.
    • Faces the sides of the thoracic vertebral bodies.
    • It bears grooves/impressions from nearby structures:
      • Right lung: esophagus and azygos vein.
      • Left lung: descending thoracic aorta.
The term “midsternal part” is likely intended to mean the anterior mediastinal part, not a separate standard surface of the lung. The hilum separates the anterior mediastinal part from the posterior vertebral part.
