MISFITS medical term mnemonic meaning

THE MISFITS is a pediatric emergency mnemonic used to recall the common causes of neonatal/infant collapse and critical illness in the Emergency Department. It's especially helpful when evaluating a sick neonate with nonspecific symptoms.

Clinical Context

  • Used in the Emergency Department for infants (typically <3 months) presenting with nonspecific collapse — poor feeding, lethargy, irritability, cyanosis, or apnea.
  • The mnemonic prevents anchoring to sepsis alone, which is a common pitfall. Before labeling a sick neonate as septic, always ask: "Could this be cardiac?"
  • Ductal-dependent cardiac lesions (e.g., hypoplastic left heart, critical aortic stenosis, coarctation of the aorta) classically present in the first 1–2 weeks of life when the ductus arteriosus closes. Treatment: prostaglandin E1 (PGE1) to reopen the duct.
  • The hyperoxia test helps distinguish cardiac from pulmonary causes of cyanosis.

Bottom line: When a neonate is sick and you don't know why — run through THE MISFITS to avoid missing a life-threatening, treatable cause.
