Imipenem is administered with cilastatin as IV imipenem-cilastatin.
Typical adult dose with normal renal function (CrCl ≥90 mL/min):
- 500 mg IV every 6 hours is a common regimen.
- Severe infections: 1 g IV every 8 hours, or 1 g IV every 6 hours.
- Do not exceed 4 g/day. Doses over 500 mg are infused over 40-60 minutes. DailyMed prescribing information
Renal impairment: reduce the dose when CrCl <90 mL/min. Incorrect dosing, particularly with CrCl ≤30 mL/min or dialysis, raises the risk of seizures. In hemodialysis, it is usually given after dialysis, using the renal-adjusted regimen.
Pediatric patients ≥3 months: usually 15-25 mg/kg IV every 6 hours, with dosing capped and adjusted by age, weight, infection severity, and kidney function.
The appropriate regimen depends on infection site, organism susceptibility, body weight, kidney function, and whether this is conventional imipenem-cilastatin or imipenem-cilastatin-relebactam (a different product and dose). Please provide the patient’s age/weight, indication, serum creatinine or CrCl, and dialysis status for a safe specific dose.
