HER2 Reporting - Major Update

Following the DESTINY-Breast 04 and 06 trials, HER2 categories are now expanded:
  • HER2-positive (IHC 3+ or ISH-amplified)
  • HER2-low (IHC 1+ or IHC 2+/ISH-non-amplified)
  • HER2-ultralow (IHC 0 with faint/incomplete membrane staining in >0% but ≤10% of cells) - newly defined; eligible for trastuzumab deruxtecan (T-DXd)
  • HER2-null/zero (IHC 0 with no membrane staining at all) The term 'variant' is now reserved exclusively for molecular/genetic alterations (not morphologic subtypes).

New Diagnostic Entity

  • ILC with extracellular mucin - recognized as a distinct subtype with specific prognostic implications.

Mucinous Carcinoma - Restricted Definition
