Pre-Class Review: Organization of Upper Limb, Pectoral Region & Clavicle


PART 1 - Organization of the Upper Limb

The upper limb is divided into these named regions - your teacher will almost certainly point to these:

The big concept: The upper limb is attached to the trunk only through the clavicle (bony link) and muscles. Everything - all nerves, vessels, lymphatics - passes through the axilla to reach the arm.

PART 2 - The Clavicle

Right Clavicle - Superior, Anterior and Inferior views from Gray's Anatomy for Students
What it is: The only bony link between the trunk and upper limb. Fully palpable along its entire length.

Shape

  • S-shaped bone
  • Medial part - convex forward (like a "C")
  • Lateral part - concave forward (curves back)

Two ends - know these cold:

Key landmarks on the inferior surface (lateral third):

  • Conoid tubercle - for the conoid part of coracoclavicular ligament
  • Trapezoid line - for the trapezoid part of coracoclavicular ligament
  • These two together = coracoclavicular ligament - suspends the scapula from the clavicle

Muscle attachments (teacher WILL ask this):

Clinical note on clavicle:

  • Most commonly fractured bone at its middle third (junction of medial 2/3 and lateral 1/3) - because this is the narrowest and weakest point, with no muscle reinforcement
  • Fracture mechanism: fall on outstretched hand (FOOSH)

PART 3 - Pectoral Region

Muscles and Fascia of the Pectoral Region - Gray's Anatomy for Students
What it is: The region external to the anterior thoracic wall that anchors the upper limb to the trunk. Has two compartments:
  • Superficial - skin, superficial fascia, breast
  • Deep - muscles

The 3 Muscles (absolutely learn these):

1. Pectoralis Major (largest, most superficial)

  • Origin: Medial half of clavicle (clavicular head) + sternum + costal cartilages 1-7 + external oblique aponeurosis (sternal head)
  • Insertion: Lateral lip of intertubercular sulcus (bicipital groove) of humerus
  • Action: Adduction, medial rotation, and flexion of arm at shoulder
  • Nerve: Medial AND lateral pectoral nerves (both)
  • Note: fibers converge into a flat tendon - the upper fibers and lower fibers actually twist on each other

2. Pectoralis Minor (deep to major)

  • Origin: Anterior surfaces of ribs 3, 4, 5
  • Insertion: Coracoid process of scapula
  • Action: Depresses tip of shoulder; protracts scapula
  • Nerve: Medial pectoral nerve

3. Subclavius (smallest, deepest)

  • Origin: Rib I at its junction with costal cartilage
  • Insertion: Inferior surface of middle third of clavicle
  • Action: Pulls clavicle medially - stabilizes the sternoclavicular joint; depresses shoulder
  • Nerve: Nerve to subclavius (from upper trunk of brachial plexus, C5,C6)
  • Clinical importance: protects subclavian vessels if clavicle fractures

Clavipectoral Fascia - important concept:

A sheet of deep fascia that encloses the subclavius and pectoralis minor muscles and connects to the clavicle above and axilla floor below. Nerves, vessels, and lymphatics pierce it to pass between the pectoral region and axilla.

PART 4 - Key Nerves & Vessels (they may ask)
