Lithium dosing frequency depends on the formulation and your prescriber's plan:
  • Immediate-release lithium carbonate tablets/capsules: commonly taken 2 to 3 times daily.
  • Extended-release or controlled-release tablets: often taken twice daily, and sometimes as a single nighttime dose if specifically prescribed.
The dose and frequency must be individualized using 12-hour blood lithium levels, kidney function, age, other medicines, and side effects. Do not change the schedule, skip doses, double a missed dose, or switch between immediate- and extended-release products without the prescriber’s direction. If you miss a lithium dose:
  • Take it when you remember only if it is still well before your next scheduled dose. Some lithium guidance uses a cutoff of within 3 hours of the usual dose time.
  • If it is close to the next dose, skip the missed dose and take the next one at the usual time.
  • Never double up or take an extra dose to compensate.
  • If you miss more than one dose, or are unsure because of your particular schedule or lithium formulation, contact your pharmacist or prescriber for individualized advice.
Do not stop lithium suddenly without medical guidance. Keep your fluid and salt intake reasonably consistent, and seek urgent help if you develop toxicity symptoms such as severe tremor, vomiting or diarrhea, confusion, slurred speech, or trouble walking.
