For open appendicectomy, the standard incision is the right iliac fossa gridiron (McBurney) incision.
- It is centered on McBurney’s point: at the junction of the lateral one-third and medial two-thirds of a line from the right anterior superior iliac spine (ASIS) to the umbilicus.
- The incision is oblique, roughly perpendicular to the ASIS-umbilicus line.
- Muscles are split in the direction of their fibres.
A common alternative is the Lanz incision, a transverse right lower-quadrant incision at the same general level. It follows skin tension lines and may give a better cosmetic scar. NCBI surgical-incision reference
For laparoscopic appendicectomy, access is usually through an umbilical port plus two small lower-abdominal ports, rather than a McBurney incision.
