A dynamic pituitary MRI is done mainly to find very small pituitary tumors, especially microadenomas smaller than 10 mm.
During the scan, gadolinium contrast is injected and rapid images are taken every few seconds. The normal pituitary gland receives contrast and enhances quickly, while many adenomas enhance more slowly. For a short time, the tumor appears as a darker small area against the bright normal gland.
It is particularly useful when doctors suspect:
Cushing disease from an ACTH-secreting microadenoma
High prolactin suggesting a prolactinoma
Acromegaly from a growth-hormone-secreting adenoma
A pituitary lesion not clearly seen on a routine MRI
Residual or recurrent tumor after pituitary surgery
It also helps distinguish normal pituitary tissue from a lesion and assists surgical planning. The rapid series is typically acquired about every 10 to 15 seconds after contrast injection. Not everyone needs it. Larger tumors are often visible on standard contrast MRI, and the radiologist/endocrinologist selects dynamic imaging when a small lesion is suspected.
