Your description is less typical of classic jock itch (tinea cruris) because that often has a scaly, raised, clearer outer edge and usually spares the scrotum. Itching of the scrotal skin plus groin folds, worsened by sweat and friction, with peeling and redness after rubbing can fit intertrigo: irritation from moisture/friction, sometimes with yeast (Candida). Contact dermatitis, eczema, psoriasis, and erythrasma are other possibilities. A clinician can distinguish these, sometimes with a simple skin scraping test. Merck’s diagnostic guidance notes that scrotal inflammation favors candidal intertrigo or another dermatitis over standard tinea cruris.
What you can do now:
  • Keep the area dry: gently pat dry after bathing and sweating. A cool hairdryer setting can help. Do not rub.
  • Wear loose, breathable cotton underwear and change it promptly after sweating. Avoid tight synthetic clothing.
  • Use only mild, fragrance-free soap or just water. Avoid perfumes, deodorant sprays, antiseptics, and harsh scrubbing in the groin.
  • Because yeast is possible, an over-the-counter azole antifungal such as clotrimazole 1% or miconazole 2% can be applied as a thin layer to the affected fold skin twice daily for 2 weeks. Stop if it clearly burns or worsens the rash.
  • Do not use steroid-antifungal combination creams or steroid cream alone unless a clinician examines you. Steroids can worsen or disguise fungal infection and thin sensitive scrotal skin.
  • Do not share towels. Wash underwear and towels regularly. If you have scaling or itching between the toes, get that treated too, since athlete’s foot can re-seed groin fungal infections.
Please arrange an in-person visit with a primary-care clinician or dermatologist if it is not clearly improving within 7-14 days, keeps returning, spreads, or you are unsure of the diagnosis. Go sooner if there is significant pain, swelling, pus, open sores, fever, blisters, painful urination, penile discharge, or rapidly spreading redness. Recurrent groin-fold rashes also warrant checking for contributing factors such as diabetes, excess sweating, or new products/clothing exposures.
Textbook guidance similarly notes that tinea cruris generally spares the scrotum, while scrotal involvement, erosions, or small nearby pustules should raise suspicion for cutaneous candidiasis; keeping folds dry and wearing loose clothing help prevent recurrence. clotrimazole 1% cream directions groin skin twice daily official drug label

Use clotrimazole 1% cream:
  • Apply a very thin film, just enough to lightly cover the itchy/red skin in the groin folds and affected external scrotal skin. You do not need a thick layer.
  • Use twice daily, morning and night, after gently washing and fully patting the area dry.
  • Continue for 2 weeks. If it starts improving, finish the 2 weeks unless the product label or a clinician tells you otherwise.
  • Use it on external skin only. Do not put it inside the penis opening, anus, or on deep/open/raw areas.
  • Wash your hands before and after application.
