Ab is spirometer se hum 4 basic volumes measure kar sakte hain. Chaliye ek ek karke dekhte hain."

1. TIDAL VOLUME (TV)
"Sabse pehle - Tidal Volume. TV = 500 mL.
Ye hai normal quiet breathing ka volume - jab aap bina kisi extra effort ke saans lete ho. Na zyada andar, na zyada bahar. Bas normal breath.
Remember: TIDAL - jaise tide aata hai, normal, ek rhythm mein. 500 mL."
2. INSPIRATORY RESERVE VOLUME (IRV)
"Ab maan lo main aapko bolta hoon - 'Ek deep breath lo - jitna ho sake utna andar lo.' Toh jo extra air aap andar le sakte ho apni normal TV ke upar - usko bolte hain Inspiratory Reserve Volume.
IRV = 3000 mL - teen litre!
Ye aapka 'reserve' hai - emergency mein ya exercise mein use hota hai."
3. EXPIRATORY RESERVE VOLUME (ERV)
"Similarly - ab main bolta hoon 'Poora bahar nikalo saans - jitna ho sake force se.' Jo extra air bahar nikal sakte ho normal expiration ke neeche - wo hai Expiratory Reserve Volume.
ERV = 1200 mL
Ye bhi ek reserve hai - forced expiration ke liye."
4. RESIDUAL VOLUME (RV)
"Ab ye most interesting wala hai - Residual Volume.
Jab aapne poori force se saans bahar nikaal di - kya lungs bilkul empty ho gaye? NAHI!
Lungs mein hamesha kuch air bachi rehti hai - aap chahe kitna bhi force karo. Is bachi hui air ko kehte hain Residual Volume.
RV = 1200 mL
Aur yahan ek bahut important point hai - RV ko hum SPIROMETER SE MEASURE NAHI KAR SAKTE! Kyunki spirometer sirf whi air measure karta hai jo bahar aati hai. Jo andar hi reh jaaye - wo measure nahi hogi.
RV measure karne ke liye special techniques chahiye - Helium Dilution method ya Body Plethysmograph."

1. INSPIRATORY CAPACITY (IC)
"IC = TV + IRV
= 500 + 3000 = 3500 mL
Ye hai maximum volume jo aap resting position se inspire kar sakte ho."
2. FUNCTIONAL RESIDUAL CAPACITY (FRC)
"FRC = ERV + RV
= 1200 + 1200 = 2400 mL
Ye bahut important hai! Jab aap normal quiet expiration karte ho - aur phir ruk jaate ho - us waqt lungs mein jo air bachi hai - wo FRC hai.
Isko kehte hain - 'Resting volume' ya 'Equilibrium volume' of lungs.
Think of it like this - ye lungs ka 'neutral gear' hai. Na fully inflate, na fully deflate.
FRC bhi spirometer se measure nahi hoti - kyunki RV include hai isme."
3. VITAL CAPACITY (VC)
"VC = IRV + TV + ERV
= 3000 + 500 + 1200 = 4700 mL
Ye hai maximum air jo ek deep breath ke baad aap force se bahar nikal sakte ho.
Physiotherapy ke liye ye MOST IMPORTANT measurement hai! Ye directly aapke patients mein measure ki jaati hai.
VC badhti hai - bade body size mein, males mein, athletes mein.
VC kam hoti hai - age ke saath, lung diseases mein, obesity mein.
Aapke COPD patients, post-surgical patients - inka VC monitor karna aapki zimmedari hogi."
4. TOTAL LUNG CAPACITY (TLC)
"TLC = VC + RV
= 4700 + 1200 = 5900 mL (approx 6 litres)
Ye hai total volume of lungs at maximum inspiration - bilkul poora bharke.
TLC bhi spirometer se nahi naapte - RV include hai."

"Normal saans mein sirf do muscles kaam karti hain:
1. Diaphragm - HERO MUSCLE!
Diaphragm sabse important muscle hai - 75% breathing yahi karta hai.
Ye ek dome-shaped muscle hai - bilkul ek umbrella ki tarah - jo lungs ke neeche hoti hai.
Jab ye contract karti hai - ye FLAT ho jaati hai - neeche dhans jaati hai. Isse chest ka vertical diameter badh jaata hai.
Innervation yaad rakho - Phrenic Nerve - C3, C4, C5. Ek mnemonic - 'C3, C4, C5 keeps the diaphragm alive!'
Physiotherapy relevance - agar C4 level pe spinal cord injury hogi - diaphragm paralyzed - patient khud saans nahi le payega - ventilator chahiye.
2. External Intercostal Muscles
Ye ribs ke beech mein hoti hain - bahar ki taraf.
Ye contract karti hain aur ribs ko upar aur bahar uthati hain - isse chest ka antero-posterior aur lateral diameter badhta hai.
Pump handle movement - sternum utha - AP diameter badha.
Bucket handle movement - ribs uthein - lateral diameter badha."
PRESSURE CHANGES - Inspiration mein:

Muscles contract kari → Chest bada hua → Volume badha → Boyle's Law se Pressure kam hua → Intrapulmonary pressure atmospheric se NEECHE chali gayi → Air ANDAR aa gayi!
Yahi mechanism hai. Simple!"

FORCED INSPIRATION - Accessory muscles:

- Sternocleidomastoid (SCM) - sternum aur clavicle ko utha deta hai
- Scaleni muscles - upper ribs uthata hai
- Pectoralis minor, Serratus anterior
Physiotherapy tip - COPD patients aksar ye accessory muscles bahut use karte hain. Aap clinic mein dekhoge - unke SCM aur scaleni hypertrophied hote hain. Ye sign hai ki unhe breathing ke liye extra mehnat karni pad rahi hai."

QUIET EXPIRATION - Passive!
"Aur ab sabse important concept - quiet expiration mein koi muscle kaam NAHI karti!
Haan - bilkul sach. Aap abhi saans lete ho - andar jaata hai saans - phir bahar aata hai - wo bahar aana khud ho jaata hai. Koi effort nahi.
Kyun? Kyunki lungs elastic hote hain - jaise ek rubber balloon. Jab balloon ko inflate karo aur choddo - wo khud apne aap deflate hota hai. Waise hi lungs bhi.
Elastic recoil of lungs → Chest chhota hua → Pressure badha → Air bahar gayi.
Expiration is PASSIVE at rest."
FORCED EXPIRATION - Active:
"Jab aapko force se saans bahar nikalna ho - jaise singing, blowing, coughing:
1. Internal Intercostal Muscles:
Ye ribs ko neeche push karti hain - chest compress hota hai.
2. Abdominal Muscles:
Rectus abdominis, obliques, transversus abdominis - ye contract karke diaphragm ko upar dhakelte hain - thoracic volume aur bhi kam hoti hai."
