pediatric fentanyl continuous infusion dose mcg kg hour hospital guideline
For a ventilated pediatric patient in ICU, a common IV fentanyl continuous infusion range is:
Start: 1 microgram/kg/hour
Titrate: 1-10 micrograms/kg/hour to analgesia/sedation response and adverse effects
A loading dose, if clinically appropriate, is often 2-3 micrograms/kg IV given slowly. PCH pediatric monograph
For critically ill children, some PICU protocols use 2-4 micrograms/kg/hour after a 2 micrograms/kg bolus when fentanyl is selected for sedation. RCH PICU guideline
Neonates are different: one neonatal protocol lists 1-5 micrograms/kg/hour, but local NICU guidance should be followed. Neonatal fentanyl protocol
Use only with continuous cardiorespiratory monitoring and readiness to manage apnea, hypoventilation, hypotension, and chest-wall rigidity. Rapid IV administration particularly increases rigidity risk in children. This must be prescribed and titrated under a pediatric ICU/anesthesia or institutional protocol.
