Short answer: No, avoid it
Ibuprofen (and other NSAIDs, plus aspirin) should not be used for fever/pain in dengue. Paracetamol (acetaminophen) is the recommended antipyretic/analgesic instead.
Why
Dengue causes thrombocytopenia, platelet dysfunction, and increased vascular permeability, all of which raise bleeding risk. NSAIDs and aspirin add on top of this by:
- Inhibiting platelet aggregation (antiplatelet effect), further impairing clotting
- Irritating the gastric mucosa, increasing risk of GI bleeding
- Aspirin specifically carries an added theoretical risk of Reye syndrome in children with viral illness
Multiple standard references converge on this:
- "Do not give acetylsalicylic acid (aspirin), ibuprofen or other non-steroidal anti-inflammatory agents (NSAIDs) as these drugs may aggravate gastritis or bleeding... Massive bleeding may occur... when acetylsalicylic acid, ibuprofen or corticosteroids have been taken." - Park's Textbook of Preventive and Social Medicine

The CDC's patient guidance is direct: "Take acetaminophen to control fever and relieve pain. Do not take aspirin or ibuprofen."
A caveat worth knowing
A 2025 retrospective observational study found that low-dose ibuprofen (used briefly for fever) had a bleeding/safety profile similar to acetaminophen in non-severe dengue patients (Liang et al., J Med Virol 2025, PMID 40708402), and a 2018 discussion paper questioned whether the blanket NSAID contraindication is overly cautious in mild, non-hemorrhagic cases (PMID 30575425). These are exploratory findings, not practice-changing evidence, and they don't apply to warning-sign or severe dengue where bleeding risk is real. Guidelines (WHO, CDC, AAP) have not changed their recommendation.
Bottom line: stick with paracetamol for fever/pain in suspected or confirmed dengue, avoid ibuprofen/NSAIDs/aspirin, and watch for warning signs (bleeding, abdominal pain, persistent vomiting, lethargy) especially as fever subsides, which signal a need for urgent medical evaluation.
