Epididymal cyst removal is generally considered a minor urological operation, not major surgery.
It is usually performed through a small scrotal incision, commonly under general or spinal anaesthesia, and many people go home the same day. The procedure does still involve an operating theatre and anaesthesia, so it should not be treated as trivial. The BAUS patient information describes these usual anaesthetic options and possible complications.
Important considerations:
  • It may cause temporary scrotal swelling, bruising, and pain.
  • Recovery from routine activities is often about 1-2 weeks, while heavy exercise/straining may need longer.
  • There is a small risk of infection, bleeding/haematoma, chronic scrotal pain, recurrence, or injury/scarring of the epididymis.
  • Fertility can be affected, particularly if surgery scars or obstructs the epididymis. Discuss this carefully with the urologist if you want children in the future.
The operation can become more involved if the cyst is very large, recurrent, infected, difficult to separate from the testicle/epididymis, or if there are significant medical conditions or blood-thinning medicines.
