Emergency (go to ED / urgent neurosurgical evaluation) — think pituitary apoplexy

Pituitary apoplexy is sudden hemorrhage or infarction of a pituitary adenoma (often a previously unknown macroadenoma). Send for immediate evaluation if any of these occur, especially in someone with a known or suspected pituitary tumor:
  • Sudden, severe headache - classically retro-orbital, bifrontal, or suboccipital (present in 63-100% of cases) - Tintinalli's Emergency Medicine
  • Acute visual field defects or vision loss (up to 75% of patients) - classically bitemporal hemianopia from chiasm compression

Not urgent on their own

Isolated mild prolactin elevation, irregular periods without visual/neurologic symptoms, or occasional galactorrhea without headache or vision change can be worked up on a routine outpatient basis (repeat level, TSH, pregnancy test, medication review, macroprolactin assay as discussed previously).
Bottom line: the red flags that turn a prolactin workup into an emergency are sudden severe headache, visual field loss or double vision, and altered mental status - these suggest pituitary apoplexy and need immediate care.
