American Association of Endodontists pain swelling after root canal treatment patient emergency

Pain and swelling beneath a crown on a root-canal-treated tooth usually suggests infection around the root, leakage under the crown, a missed canal, or a crack. It needs an urgent dental examination, ideally by an endodontist.
The dentist will usually take an X-ray and check the crown, gum, bite, and tooth for fracture. Treatment may be:
  1. Root canal retreatment: the crown may be removed or accessed through, the canals cleaned again, disinfected, and resealed.
  2. Drainage if there is an abscess or pus collection.
  3. Apicoectomy: minor surgery to remove infection at the root tip if retreatment is not suitable or fails.
  4. Extraction and replacement if the tooth is badly cracked or cannot be saved.
Antibiotics are not usually the main cure for a localized tooth abscess. They may be prescribed when swelling is spreading, there is fever or feeling unwell, or the person is medically high-risk. The source of infection still needs dental treatment. Root-canal treatment works by cleaning, disinfecting, and sealing the root canals, as explained by the American Association of Endodontists.
Seek emergency care now if you have facial or neck swelling, fever, trouble swallowing, trouble breathing, cannot open your mouth normally, or swelling spreading toward the eye.
Until you are seen: avoid chewing on that side, use a cold compress externally, and use an over-the-counter pain reliever only if it is normally safe for you and according to the package directions. Do not put aspirin or other tablets directly on the gum.
Recent research also recognizes post-endodontic pain and swelling as an “endodontic flare-up,” but it does not replace assessment for persistent infection or a fracture (systematic review: PMID 42026978).
