Here's a practical guide to operating the Hamilton T1 ventilator as shown on your screen:

Hamilton T1 — How to Use

What You're Looking At (Current Screen)

The device is running SIMV+ mode (Adult/Ped.) with these active settings:
ParameterValue
ModeSIMV+ (Adult/Ped.)
Tidal Volume (Vt)450 mL
PEEP/CPAP5 cmH₂O
FiO₂100%
Ppeak24 cmH₂O
Exp Minute Volume6.5 L/min
VTE (exp tidal vol)309 mL
fTotal (total rate)17 breaths/min
⚠️ Active alert: "Battery 2: Replacement required" — the backup battery needs replacement. Connect to AC power (the AC icon is shown at the bottom right of the display).

Starting Ventilation — 6 Key Steps

  1. Press one of the three Quick Setup buttons (on the touchscreen) — these auto-populate settings based on patient type.
  2. Select Male or Female — the ventilator uses sex to calculate Ideal Body Weight (IBW).
  3. Set Patient Height — use the Press-and-Turn knob (the large green dial on the right side). The ventilator calculates IBW from height and sex, which drives startup settings.
  4. Select the Mode — tap the "Modes" button (top right of screen). SIMV+ is the default. Other common options:
    • CMV+ — equivalent to Assist Control/Volume (recommended for most ICU/transport patients)
    • PCV+ — pressure-controlled
    • ASV — adaptive support ventilation (auto-adjusts to patient effort)
  5. Review Controls and Alarms — tap the "Controls" button to adjust Vt, RR, PEEP, FiO₂, I-time; tap "Alarms" to set limits.
  6. Touch "Start Ventilation" — confirm to begin.

Adjusting Settings

All parameter changes are made via the Press-and-Turn knob:
  • Tap the parameter on the touchscreen to select it
  • Turn the knob to adjust the value
  • Press the knob (or tap "Accept") to confirm
Key controls accessible via the "Controls" tab:
  • Vt — target tidal volume (aim ~6–8 mL/kg IBW; lung protective = 6 mL/kg)
  • RR — respiratory rate
  • PEEP — typically 5 cmH₂O; higher in ARDS
  • FiO₂ — start at 100%, wean as tolerated
  • I-Time — sets inspiratory time; the I:E ratio is NOT set directly — it changes automatically when you adjust RR or I-time. If you increase RR, manually adjust I-time to maintain your target I:E ratio (e.g., 1:2)

Ventilation Modes Explained

ModeEquivalentUse
CMV+ (APVcmv)Assist Control/Volume (PRVC)Most adult patients; volume-targeted with adaptive pressure
SIMV+ (APVsimv)SIMV/VolumeSpontaneously breathing patients; mandatory breaths + pressure-support for spontaneous breaths
PCV+Pressure ControlWhen peak pressure is the concern
PSIMV+Pressure SIMVSIMV with pressure-controlled mandatory breaths
ASVAdaptive Support VentICU weaning; adjusts automatically
SPONTPSV/CPAPFully spontaneous breathing support

Navigation Buttons (Screen Bottom)

  • Monitoring — live waveforms (Paw = airway pressure in yellow, Flow in pink/magenta as shown)
  • Tools — advanced calculations, lung mechanics
  • Events — log of alarms and events
  • System — ventilator settings, screen brightness, battery

Physical Buttons (Right Side Panel)

  • Power button (top right, green ring)
  • FiO₂ quick increase (↑O₂ button — press for rapid 100% O₂ for 2 minutes)
  • 2 MIN button — manual 2-minute 100% O₂ flush
  • Alarm silence — triangle/bell icon

Important Tips

  • Always set patient height and sex first — this determines IBW-based default settings.
  • If the patient deteriorates: disconnect and manually bag with 100% O₂ while troubleshooting.
  • The T1 has an online simulator for practice — excellent for familiarizing yourself with the interface before clinical use.
  • The full Hamilton T1 Operator's Manual is available from Hamilton Medical directly.
