Tennis Elbow (Lateral Epicondylitis)

What it is: Tennis elbow is an overuse tendinopathy of the common extensor origin at the lateral epicondyle of the humerus, most often involving the extensor carpi radialis brevis (ECRB) tendon (it can also involve the extensor carpi radialis longus and extensor digitorum communis). Despite the name, it's more common in non-athletes (manual laborers, repetitive gripping/typing work) than in tennis players, with peak incidence in the early fifth decade. Risk factors include female sex, smoking, manual labor, and use of statins - Campbell's Operative Orthopaedics, 15th Ed.
Cause: Originally thought to be inflammatory, current understanding is that it's a degenerative process - repetitive supination/pronation with the elbow near full extension causes microtearing at the ECRB origin, leading to disorganized "angiofibroblastic" reparative tissue rather than true inflammation (hence "tendinosis/tendinopathy" is a more accurate term than "-itis").
Diagnosis (clinical):
  • Point tenderness just distal and anterior to the lateral epicondyle
  • Pain reproduced by resisted wrist extension and forearm supination, and pain with gripping
  • Need to distinguish from radial tunnel syndrome (tenderness 3-4 cm more distal, reproduced by resisted long-finger extension) - the two coexist in about 5% of cases - Campbell's Operative Orthopaedics, p. 2851 region.

Treatment

Step 1 - Conservative management (first-line, works in 80-95% of patients):
  • Activity modification/relative rest - avoid the repetitive wrist-extension/gripping activities that provoke pain
  • Ice and short-term oral or topical NSAIDs for pain
  • Counterforce bracing - a forearm strap ("tennis elbow brace") worn just distal to the epicondyle to offload the extensor origin
  • Wrist splint/rigid brace in mild extension to rest the extensors acutely
  • Physical therapy - eccentric wrist extensor strengthening and stretching programs have the best evidence for intermediate/long-term improvement; friction massage is also used

Step 3 - Surgery (reserved for recalcitrant cases, typically after 6-12 months of failed nonoperative treatment, roughly 5-10% of patients):
  • Open debridement/release - excision of the degenerative ECRB tissue with reattachment (Nirschl technique is the classic described procedure)
  • Arthroscopic debridement/release - increasingly preferred, allows inspection of the joint for associated intra-articular pathology
  • A 2025 systematic review/meta-analysis comparing open vs. arthroscopic debridement (Tang et al., PMID 40134024) found both are effective, with arthroscopic approaches associated with faster recovery in some outcome measures, though evidence quality varies across studies.
  • Postoperative rehab involves progressive range-of-motion and strengthening.

Practical summary

Most cases resolve with rest, bracing, NSAIDs, and eccentric strengthening exercises over weeks to a few months. Steroid injections are useful for short-term flares but not for repeated long-term use. Surgery is a last resort for the minority who fail extended conservative treatment.
