For a 4-year-old, use DTaP (diphtheria, tetanus, acellular pertussis), not Td/Tdap.

If no previous documented DTaP doses

Give:
  1. DTaP today
  2. DTaP after 4 weeks
  3. DTaP 4 weeks later
  4. DTaP 6 months after dose 3
Because dose 4 is given at age 4 years or older and at least 6 months after dose 3, a 5th DTaP dose is not needed.

If partially vaccinated

Use the documented doses to continue, not restart:

CDC tetanus does not spread person to person

If the child has tetanus, family members do not need vaccination or preventive treatment because of contact with the child.
Tetanus does not spread from person to person. It is acquired when C. tetani spores from the environment enter a wound. CDC tetanus guidance confirms it is non-contagious.
However, family members who are not vaccinated should still arrange routine tetanus vaccination for their own protection:
  • Adults with no or unknown tetanus vaccination history: 3-dose primary series: Tdap now, Td or Tdap after at least 4 weeks, and Td or Tdap 6 to 12 months later.
  • Then a Td or Tdap booster every 10 years.
  • Any family member with a dirty/deep wound should seek prompt medical care, as a tetanus-containing vaccine and sometimes tetanus immune globulin may be needed based on vaccine history.
If the child’s infection was not tetanus, tell me the diagnosis. Contact vaccination or preventive medication depends on the specific infection.
