Microbiology K. pneumoniae is a Gram-negative, non-motile, encapsulated, lactose-fermenting rod in the family Enterobacteriaceae. It colonizes the human gut (5-35% prevalence) and oropharynx (1-5%) in healthy people, and is broadly prevalent in the environment. Its thick polysaccharide capsule is antiphagocytic and is a major virulence determinant, along with siderophores (aerobactin, yersiniabactin) for iron scavenging and endotoxin (LPS).
Two clinically distinct pathotypes
  1. "Classic" K. pneumoniae (cKp) - the dominant form in Western countries, largely health care-associated (hospitals, long-term care facilities).
