Cholangitis is infection/inflammation of the bile ducts, most commonly caused by biliary obstruction (gallstones, strictures, malignancy) leading to bacterial overgrowth in stagnant bile. It presents with Charcot's triad (fever/rigors, jaundice, RUQ pain) and in severe cases Reynolds' pentad (adding shock and altered mental status). Treatment is IV antibiotics plus urgent biliary decompression (ERCP).
