With intrathecal dexmedetomidine added to bupivacaine spinal anesthesia, the block is usually prolonged:
  • Sensory block: about 2 to 2.5 hours longer on average
  • Motor block: about 1.5 to 2 hours longer
  • Postoperative analgesia: commonly prolonged by 3 to 5 hours, depending on the local anesthetic, dose, and surgery.
For example, 5 micrograms intrathecal dexmedetomidine with bupivacaine increased sensory-block duration by about 134 minutes versus bupivacaine alone in a meta-analysis (review result).
The exact duration cannot be predicted from dexmedetomidine alone. It depends especially on bupivacaine dose/baricity, intrathecal dexmedetomidine dose, patient factors, and surgical context. Intrathecal dexmedetomidine use is generally off-label, and bradycardia/hypotension should be anticipated.
