Act 1 - College & Love Arjun Reddy Deshmukh is a brilliant, hotheaded MBBS student from a wealthy family. He's the college topper but also notorious for his short temper and rule-breaking attitude. During his final years, he falls deeply in love with Preethi, a junior student. Their relationship is intense and possessive - Arjun makes it clear to everyone that Preethi is his.

Act 2 - Separation Preethi's family, who are from a conservative Tulu community, arranges her marriage to someone of their caste. Preethi, unable to stand up to her family, goes through with the marriage. Arjun is not given a chance to fight for her - she simply disappears from his life.

Act 3 - Downward Spiral Heartbroken and unable to cope, Arjun throws himself into alcohol, drugs, and casual relationships. He loses his surgical job, alienates his friends and family, and becomes completely self-destructive. His health and career hit rock bottom.

Act 4 - Redemption Arjun eventually finds out that Preethi's marriage was annulled (her husband couldn't accept her past with Arjun). After going through a painful recovery, Arjun rebuilds himself. The two are finally reunited, and the film ends with them together and having a child.

Themes: obsessive love, toxic masculinity (debated), heartbreak, self-destruction, and eventual redemption.

The film was a massive commercial hit (made on ~₹5 crore, grossed ~₹51 crore) and was later remade in Hindi as Kabir Singh (2019) with Shahid Kapoor.
