Herniorrhaphy - Short Notes
Definition: Surgical repair of a hernia.
Types of Herniorrhaphy:

| Shouldice Repair | Multi-layer fascial repair; considered gold standard among tissue repairs |
| McVay (Cooper's Ligament) Repair | Repairs floor of inguinal canal to Cooper's ligament; used for femoral hernias too |
| Modified Bassini Repair | Strengthens posterior wall of inguinal canal |
| Lichtenstein Repair | Tension-free mesh repair; most widely used today |
Important Point:
- All the above (except Lichtenstein) are tension repairs - the tissues are sutured under tension
- High recurrence rate is associated with tension repairs
- Lichtenstein uses a prosthetic mesh - making it a tension-free repair with lower recurrence
Key Distinction:
Herniorrhaphy = repair using patient's own tissue (with tension) Hernioplasty = repair using mesh (tension-free, e.g., Lichtenstein)
