Herniorrhaphy - Short Notes

Definition: Surgical repair of a hernia.

Types of Herniorrhaphy:

Shouldice RepairMulti-layer fascial repair; considered gold standard among tissue repairs
McVay (Cooper's Ligament) RepairRepairs floor of inguinal canal to Cooper's ligament; used for femoral hernias too
Modified Bassini RepairStrengthens posterior wall of inguinal canal
Lichtenstein RepairTension-free mesh repair; most widely used today

Important Point:
  • All the above (except Lichtenstein) are tension repairs - the tissues are sutured under tension
  • High recurrence rate is associated with tension repairs
  • Lichtenstein uses a prosthetic mesh - making it a tension-free repair with lower recurrence

Key Distinction:
Herniorrhaphy = repair using patient's own tissue (with tension) Hernioplasty = repair using mesh (tension-free, e.g., Lichtenstein)
