An aortic dissection does not cause acute aortic dissection. They are essentially the same condition.
Aortic dissection: a tear in the aorta's inner lining (intima) allows high-pressure blood to enter the wall and split its layers, creating a false channel, called a false lumen.
Acute aortic dissection: an aortic dissection in the early period after it begins, conventionally within 14 days of symptom onset.
The usual sequence is:
The aortic wall becomes vulnerable, often from long-standing hypertension, connective-tissue disease such as Marfan syndrome, a bicuspid aortic valve-associated aortopathy, an aneurysm, or rarely trauma/procedures.
An intimal tear, or sometimes bleeding within the aortic wall, occurs.
Blood dissects through the media, forming a false lumen. This is the acute dissection event. The ACC/AHA guideline describes this mechanism as blood entering the media through an intimal tear and splitting the aortic wall.
It can then cause life-threatening complications: rupture, aortic-valve regurgitation, cardiac tamponade, stroke, myocardial infarction, kidney or bowel ischemia, or limb ischemia.
