This is an important development. When omeprazole fails to relieve stomach burning and abdominal pain after several days, it strongly suggests the problem is not simply acid-related. Here is what this means:
Why Omeprazole May Not Be Working
Reason 1: H. pylori Infection - Acid suppression alone is NOT enough
If your mother has H. pylori bacteria in her stomach, omeprazole alone will not cure it. H. pylori causes chronic gastritis and peptic ulcers and requires a specific combination antibiotic treatment called Triple Therapy:
- Omeprazole + Amoxicillin + Clarithromycin (for 14 days)
Without the antibiotics, the infection persists and the burning continues despite taking the PPI. This is one of the most common reasons omeprazole fails.
Reason 2: Gallstones / Gallbladder Disease (Very Important)
This is frequently mistaken for stomach acidity. Key features:
- Burning/aching pain in the upper abdomen or right upper abdomen
- Nausea, bloating
- Pain often worse after fatty or oily meals
- Does NOT respond to antacids or omeprazole at all
According to Sleisenger & Fordtran's GI textbook, chronic cholecystitis causes "recurrent attacks of either steady epigastric or right upper quadrant pain" - this can feel exactly like stomach burning.
Reason 3: Functional Dyspepsia (No Structural Cause)
Over 70% of dyspepsia cases show nothing on endoscopy. Even so, the pain and burning are real. PPIs help some patients but not all - this category often needs a different class of medication (prokinetics, low-dose antidepressants).
Reason 4: Incorrect Timing of Omeprazole
Omeprazole must be taken 30-45 minutes BEFORE a meal, on an empty stomach. If taken after food or randomly, it barely works. This is a very common mistake.
Reason 5: Pancreatitis
- Pain in the upper abdomen that radiates to the back
- Nausea, vomiting
- Worsens after eating
- Completely unresponsive to omeprazole
- Requires urgent medical attention
Reason 6: Other Diagnoses That Mimic Acid
| Condition | Key Feature |
|---|
| Kidney stones | Colicky pain, may radiate to groin/back |
| Urinary Tract Infection (UTI) | Burning urination, pelvic pain, fever |
| Ovarian cyst / uterine issue | Lower abdominal pain, women |
| Intestinal infection | Diarrhea, fever |
| Gastric cancer | Weight loss, loss of appetite, older patient |
What Should Be Done NOW
Since omeprazole has failed after 4-5 days, your mother needs to see a doctor urgently for tests. Do not keep waiting.
The doctor will likely order:
- Ultrasound of the abdomen - to check for gallstones, liver, pancreas
- H. pylori test - stool antigen test or urea breath test
- Blood tests - CBC, liver enzymes, amylase/lipase (for pancreatitis), kidney function
- Urine test - to rule out UTI or kidney issue
Red Flags - Go to Emergency NOW if any of these are present:
- Severe, worsening pain
- Vomiting blood or dark "coffee ground" vomit
- Black/tarry stools
- High fever with chills
- Yellowing of skin/eyes (jaundice - suggests bile duct or liver problem)
- Inability to eat or drink
- Fainting or extreme weakness
Bottom line: 4-5 days of burning pain that is not responding to omeprazole needs a proper physical examination, blood tests, and abdominal ultrasound. Please take your mother to a doctor today - do not delay further.
