For a preterm neonate, remove the umbilical venous catheter (UVC) as soon as it is no longer needed.

Preferred practice: plan to remove or replace with a PICC at 7 days or earlier if ongoing central access is required.
Maximum duration: a UVC can remain for up to 14 days only if essential and managed with strict asepsis, but infection risk rises with longer dwell time.
Remove immediately if there is suspected catheter-related sepsis, malfunction, thrombosis, extravasation, or malposition.

The CDC NICU CLABSI guideline conditionally recommends considering UVC removal at or before 7 days. The CDC catheter recommendations allow use up to 14 days if aseptically managed.

For a preterm baby, 8 cm may be an appropriate initial UVC insertion length, especially around 1.5 to 2 kg, but it must not be fixed purely by a standard length.

A commonly used estimate is:

UVC insertion length (cm) = (1.5 × birth weight in kg) + 5.5

Examples:

1 kg: about 7 cm
1.5 kg: about 7.75 cm
2 kg: about 8.5 cm

So, 8 cm is a reasonable starting estimate for a preterm infant around 1.5 to 1.7 kg.

Confirm final tip position by X-ray or ultrasound: it should lie at the IVC-right atrial junction, usually around T8-T9 on radiograph. If the tip is in the liver/portal circulation or within the right atrium, adjust or remove it.
