Canine Babesia gibsoni treatment protocol
This is a veterinary prescription protocol. Confirm infection and species by PCR, ideally before treatment. B. gibsoni is a small Babesia and is harder to eliminate than B. canis. A dog may recover clinically yet remain a carrier.

This atovaquone-azithromycin combination is the standard recommended regimen for canine
B. gibsoni. The
CAPC Babesia guideline specifies atovaquone 13.3 mg/kg orally every 8 hours plus azithromycin 10 mg/kg orally every 24 hours, both for 10 days.
Example dose calculation
For a 20 kg dog:
- Atovaquone: 13.3 mg/kg × 20 kg = 266 mg PO every 8 hours for 10 days, with a fatty meal.
- Azithromycin: 10 mg/kg × 20 kg = 200 mg PO every 24 hours for 10 days.
The precise tablet or liquid volume must be selected by the treating veterinarian based on the available product concentration. Do not substitute a human atovaquone/proguanil combination product without veterinary direction, because formulations and tolerance differ.
Monitoring and confirmation of response

Before treatment
- CBC with platelet count, PCV/hematocrit, reticulocyte count.
- Blood smear, though a negative smear does not exclude infection.
- Species-specific PCR if possible.
- Serum biochemistry and urinalysis in clinically ill dogs.

During acute illness
- Reassess clinical status and PCV/hematocrit within a few days if anemia, jaundice, weakness, hemoglobinuria, or thrombocytopenia is present.
- Hospitalize if severe hemolysis, shock, acute kidney injury, or marked anemia develops.

After the 10-day course
- Repeat PCR at 30 days after treatment.
- Repeat PCR again at 60 days.
- If either is positive, repeat at 90 days and reassess with a veterinary internal-medicine or parasitology clinician.
- A practical definition of clearance used in a 42-dog study was two negative PCR tests 30 days apart. In that study, 39/42 dogs met this endpoint after the 10-day regimen. See the treatment and PCR follow-up study.
PCR-negative results reduce the likelihood of persistent carriage, but no protocol can promise eradication in every dog. Relapse or persistent low-level infection can occur.
Supportive treatment in clinically ill dogs
Supportive care is determined by disease severity, not used routinely in every PCR-positive dog:
- IV crystalloids if dehydration or poor perfusion is present, while avoiding fluid overload in severe anemia.
- Packed red-cell transfusion or whole blood for life-threatening anemia or clinical hypoxia.
- Antiemetic and nutritional support if vomiting/anorexia prevents oral treatment.
- Manage complications such as acute kidney injury, disseminated intravascular coagulation, or immune-mediated hemolysis under veterinary supervision.
- Avoid empiric corticosteroids unless there is a clear concurrent immune-mediated indication and the clinician has weighed the risks.
Alternative regimens
These are generally considered when atovaquone is unavailable, unaffordable, poorly tolerated, or treatment fails. Their evidence base is weaker and cure rates may be less reliable.

Key practical points
- Give every atovaquone dose with a fatty meal. Poor absorption is a major avoidable reason for failure.
- Ensure strict adherence to the every-8-hour schedule for all 10 days.
- Use effective year-round tick control. Avoid dog fights, blood donation, and breeding until infection status is resolved because B. gibsoni can spread through blood exposure and vertically.
- Do not assume that clinical recovery means the dog is noninfectious.
- There are no meaningful recent systematic reviews specific to this canine protocol. Recent PubMed records include a 2025 tafenoquine study, but it does not replace the established atovaquone-azithromycin regimen.
