Stage 1 - Embryonic (PCW 4-7)
Trachea separates from esophagus; two primary lung buds form
The cross-section shows the trachea (with its paired lumina at top) beginning to separate from the esophagus, with the respiratory primordium budding ventrally into the mesenchyme.
Stage 2 - Pseudoglandular (5-16 weeks)
Progressive branching to terminal bronchioles; glandular appearance on cross-section
Branching epithelial tubules extend into thick mesenchyme. The lung now resembles an exocrine gland in cross-section - the basis of the name. Conducting airways down to the terminal bronchioles are established by the end of this stage.
Stage 3 - Canalicular (16-26 weeks)
Type I/II pneumocyte differentiation, angiogenesis, surfactant production begins
The airway tree branches extensively and is now surrounded by a rich vascular plexus. The vasculature becomes closely apposed to the epithelium, forming the early air-blood barrier. Lamellar bodies (surfactant stores) begin forming in type II pneumocytes.
Stage 4 - Saccular (26 weeks to term)
Primary sacculi form; interstitial tissue thins; double capillary loops in primary septa
Conducting airways are visible proximally (with smooth muscle rings) and bulbous primary sacculi form distally. The vasculature fully invests the lung parenchyma. Primary septa with double capillary loops delimit individual saccular units.
Stage 5 - Alveolar (Birth onwards)
Secondary septa form true alveoli; double capillary loops thin to single loops; alveologenesis continues into early adulthood
Secondary myofibroblasts at septal tips drive ingrowth of secondary crests, generating ~480 million true alveoli. AT1 cells (covering ~95% of surface area) and AT2 cells (surfactant-producing, stem cell role) are now fully differentiated.
