A new itchy and painful red rash on the palms, face, and back should be assessed today by a clinician or dermatologist, even without fever. Possible causes include an allergic/contact reaction, scabies, a medication reaction, viral rash, or other skin inflammation. Pain and involvement of the face and palms make it hard to diagnose safely without seeing it.
Seek urgent care now if any of these occur:
Blisters, peeling, open/raw skin, purple spots, or rapid spread
Rash in the mouth, eyes, lips, or genital area
Swelling of the face/lips, breathing or swallowing difficulty
Pus, crusting, warmth, marked swelling, or worsening pain
Fever, feeling very unwell, or a new medicine in the past few weeks
Do not use new creams, cosmetics, soaps, or herbal products.
Use a cool compress and plain fragrance-free moisturizer.
Avoid scratching and keep nails short.
Do not share towels, bedding, or close skin contact if others in the household are also itchy, especially if itching is worse at night. Scabies can cause intense itch and rash and may require treatment of close contacts.
Do not apply steroid or antibiotic combination creams to the face unless prescribed.
If you can, send a clear photo and tell me: when it started, whether there are blisters or small bumps, whether anyone close to you is itchy, and any new medication, food, soap, cosmetic, or recent exposure.
