Overview

Both are refractive errors - conditions where the eye cannot focus light precisely on the retina. They differ in the direction of the focusing defect.

Mechanism (from Medical Physiology textbook)

"Myopia, or nearsightedness, occurs when the eye is too long; distant objects focus in front of the retina and appear blurred. Hyperopia (or hypermetropia), or farsightedness, is a feature of eyes that are too short; even with the lens fully accommodated, a near object focuses behind the retina and appears blurry."

How Correction Works

  • Myopia - concave lenses diverge incoming light rays, effectively moving the focal plane backward onto the retina.
  • Hypermetropia - convex lenses converge incoming light rays, moving the focal plane forward onto the retina.

Surgical Correction Options (from Kanski's Clinical Ophthalmology)

Myopia:
  • Surface ablation (PRK/LASEK) for low-moderate degrees
  • LASIK for moderate to high myopia
  • SMILE (refractive lenticule extraction) for myopia and myopic astigmatism
  • Phakic lens implants or clear lens exchange for very high refractive errors
Hypermetropia:
  • LASIK (up to 3-4 D)
  • Surface ablation for low-moderate degrees
  • Clear lens extraction or phakic lens implants for higher degrees
  • Kanski's note: "Intraocular surgical procedures are the only options for high degrees of refractive error"

Associated Conditions

  • Myopia can be associated with strabismus, retinal detachment risk (especially high myopia), and POAG
  • Hypermetropia is associated with primary angle-closure glaucoma (shallow anterior chamber due to short eyeball) and accommodative esotropia in children
