Is This Abnormal?
Yes. The normal reference range for Free T3 is approximately 2.3 - 4.2 pg/mL (some labs cite 1.5 - 6.0 pg/mL, but most standardized references place the lower limit around 2.0 - 2.3 pg/mL). A value of 1.08 pg/mL is clearly low, regardless of which lab's reference interval you use.
Note: If this value is in pmol/L, the normal range is ~3.5 - 6.5 pmol/L, and 1.08 pmol/L would still be significantly low. Always confirm the units with the reporting laboratory.
What Does a Low Free T3 Mean?
Low Free T3 can arise from several distinct mechanisms:
1. Primary or Overt Hypothyroidism
- The thyroid gland is underproducing hormone
- Typically accompanied by elevated TSH and low Free T4
- As disease progresses, T4 falls first; T3 falls last due to preferential T3 secretion by the failing gland
- Henry's Clinical Diagnosis: "Decreased T3 levels are seen in patients with severe hypothyroidism, whose T4 < 2 µg/dL"
2. Sick Euthyroid Syndrome / Nonthyroidal Illness (NTI)
- The most common cause of a low Free T3 in hospitalized/ill patients
- Any acute or chronic illness (infection, MI, surgery, malnutrition, liver disease, kidney disease) impairs peripheral conversion of T4 to T3 via 5'-deiodination
- Pattern: Low T3, normal or low T4, normal or low TSH, elevated reverse T3 (rT3)
- Harrison's (2025): "The most common hormone pattern in SES is a decrease in total and unbound T3 levels (low T3 syndrome) with normal levels of T4 and TSH. The magnitude of the fall in T3 correlates with the severity of the illness."
- This is considered an adaptive response - limiting catabolism during illness
3. Impaired T4 → T3 Conversion (Drug-Induced)
Medications that block peripheral conversion of T4 to T3:
- Amiodarone - major cause
- High-dose propranolol
- Glucocorticoids (dexamethasone, prednisone)
- These cause low T3/FT3 with normal or even elevated T4/FT4 and normal TSH
4. Chronic Kidney Disease (CKD)
- Most patients with end-stage kidney disease have decreased plasma Free T3 ("low T3 syndrome"), primarily from diminished peripheral T4-to-T3 conversion
- Low T3 in CKD is independently associated with impaired cardiac function, coronary artery calcification, increased mortality
5. Malnutrition / Fasting / Caloric Restriction
- Fasting alone can suppress T3 - this is why the low T3 state in illness is considered adaptive (limits catabolism)

To interpret a Free T3 of 1.08 in context, the following tests are essential:
