ESR (erythrocyte sedimentation rate) measures how far red blood cells settle in a vertical tube over 1 hour, reported as mm/hour. Inflammation increases proteins such as fibrinogen, causing red cells to aggregate and settle faster, so the ESR rises.
Diagnostic value
ESR is a nonspecific marker of inflammation. It cannot diagnose a particular disease by itself, but it can support a clinical diagnosis and help monitor disease activity.
It may be raised in:
- Acute or chronic infections, including tuberculosis
- Autoimmune/inflammatory diseases: rheumatoid arthritis, SLE, vasculitis
- Giant cell arteritis and polymyalgia rheumatica, where it is particularly useful, though a normal ESR does not fully exclude the diagnosis
- Some cancers, especially multiple myeloma, lymphoma, and metastatic malignancy
- Tissue injury or necrosis
Monitoring: ESR is useful for following the course of chronic inflammatory disorders. A falling ESR often supports improvement, but it changes relatively slowly compared with CRP.
Limitations
A high ESR does not necessarily mean infection. It may also increase with:
- Anaemia
- Pregnancy
- Older age
- Kidney disease
- High immunoglobulin levels
ESR may be lower than expected in polycythaemia, marked leukocytosis, abnormal red-cell shapes such as sickle cells/spherocytes, or low fibrinogen.
Therefore, interpret ESR with the history, examination, CBC, and often CRP, which responds faster to acute inflammation.
Normal values
Reference intervals vary by laboratory and method. Common Westergren values are:
