OBSERVATIONS: Acute lacunar infarct in left thalamus. Mild ischemic changes bilateral periventricular region. Rest of the brain parenchyma is normal. No focal lesions. Ventricular system appears normal. Bilateral cerebellar hemispheres are of normal morphology and signal intensity. Medulla appears normal; cerebellar peduncles appear normal. Pons and mid brain show no abnormality. No extra axial collections; Pituitary gland appear of normal size and signal intensity. Craniovertebral junction appears normal. Calvarium showing normal signal intensity. Visualised sections of orbits and paranasal sinuses show no significant abnormality. MR Angiography of circle of willis: Posterior Circulation: Bilateral vertebral arteries and the basilar artery appear normal. Left p1 segment is hypoplastic rest of the bilateral posterior cerebral arteries appear normal. Anterior Circulation: Bilateral internal carotid arteries and middle cerebral arteries, including M2 branches, appear normal. Right a1 segment is hypoplastic rest of the bilateral anterior cerebral arteries appear normal. Cervical spine : Screening images of cervical spine show degenerative disc disease at lower cervical region with mild disc bulge at C5-6 and C6-C7 level, spina cord craniovertebral junction appear normal. IMPRESSION: Acute lacunar infarct in left thalamus. Mild ischemic changes bilateral periventricular region. No significant abnormality noted in the MRA of brain except for normal variations.
