Suggested examination question

A dead body is brought for medicolegal post-mortem examination with an alleged history of hanging. Describe the external and internal findings to be recorded, preservation of exhibits, and the final opinion.

Important note

A post-mortem report is a medicolegal document. Do not enter invented facts in an actual report. The model below is an academic sample for an alleged hanging case. Replace every bracketed item with actual observations, measurements, identification details, and laboratory findings. The manner of death, such as suicide, is decided from the complete investigation, not from post-mortem findings alone.
A forensic autopsy should document external examination, internal examination, and any samples preserved for further testing, as described in StatPearls forensic-autopsy guidance. In hanging, carefully record the ligature material, knot, direction, level, continuity, width, colour, and base of the ligature mark. A typical mark may be oblique and parchmentised, but variations occur. P. C. Dikshit Textbook of Forensic Medicine and Toxicology, Fig. 23.5 and associated text.

forensic pathology hanging cause of death opinion manner of death autopsy documentation

Remarks by Medical Officer / Opinion

Opinion reserved pending receipt of the Chemical Examiner/FSL report, if viscera and other samples have been preserved.
On completion of post-mortem examination, the following opinion may be recorded:
In my opinion, death was due to asphyxia as a result of ante-mortem hanging.
The ligature mark and internal neck findings are consistent with hanging.
There is no evidence of any other fatal injury or natural disease detectable at post-mortem examination.
Final opinion is subject to the report of Chemical Examiner/FSL, if applicable.

If chemical analysis is not required or is negative

In my opinion, death was due to asphyxia consequent upon ante-mortem hanging.

If the ligature mark is present but findings are incomplete or doubtful

Use a reserved opinion rather than a definite conclusion:
The cause of death is kept pending awaiting the report of Chemical Examiner/FSL and histopathological examination, if any.
The post-mortem findings show a ligature mark over the neck. A final opinion will be furnished after considering the laboratory reports, case circumstances, and investigation findings.

If toxicology later detects poison/alcohol/drugs

Do not alter the original opinion without correlating the concentration and case facts. Add:
The final cause-of-death opinion is subject to correlation with the Chemical Examiner/FSL report and investigation findings.

Manner of death

Do not write “suicide due to hanging” solely on the basis of autopsy. The appropriate wording is:
The manner of death is to be determined by the investigating agency/court after considering scene findings, inquest papers, witness statements, ligature, and other investigation evidence.
The cause of death should state both the mechanism and precipitating cause, such as “asphyxia due to ante-mortem hanging,” rather than “asphyxia” alone. Parikh’s Textbook of Medical Jurisprudence, Forensic Medicine and Toxicology, section on certification of cause of death.
