Carcinomas usually spread first through lymphatic channels because they arise from epithelial tissues, which are richly supplied by lymphatic capillaries.
Why lymphatic spread is relatively easy:
  • Thin, permeable lymphatic walls: Lymphatic capillaries have a thin endothelial lining, incomplete basement membrane, and overlapping endothelial cells. Tumor cells can enter them more easily than blood vessels.
  • Low-pressure flow: Lymph flows slowly and at low pressure, so detached carcinoma cells are less exposed to shear stress than in the bloodstream.
  • Lymphatics drain tissues to regional nodes: Once tumor cells invade local lymphatics, they are carried along the normal drainage route to the nearest, or sentinel, lymph node.
  • Tumor-induced lymphangiogenesis: Many carcinomas produce factors such as VEGF-C and VEGF-D, which promote formation and dilation of lymphatic vessels, facilitating entry and transport of cancer cells. A review of lymphatic-metastasis mechanisms discusses this process.
This is a tendency, not an absolute rule. Carcinomas can also spread hematogenously, especially at later stages, often to liver, lung, bone, or brain. In contrast, many sarcomas more often spread through blood vessels because they arise from mesenchymal tissue.
Exam line:
Carcinomas characteristically metastasize via lymphatics because lymphatic vessels are abundant in epithelial tissues, thin-walled, low-pressure, and readily invaded by tumor cells.

Primary tumors of the central nervous system, especially brain tumors, generally do not produce regional lymph-node metastases.
Traditional exam reason: the brain parenchyma has no conventional lymphatic vessels, and CNS tumors rarely spread outside the CNS.
Important caveat: Cervical lymph-node metastasis can occur very rarely, particularly after neurosurgery, shunting, or disruption of tissue barriers. Also, meningeal lymphatic vessels are now recognized, so “no lymphatics in the brain” is an oversimplification.
One-line answer: Primary CNS tumors rarely metastasize to lymph nodes.
