FDA pregabalin prescribing information renal dosage tramadol renal impairment label

FDA labeling भी creatinine clearance under 30 mL/min में dose reduction की बात करती है. Tramadol renal warning
Bottom line:
यदि creatinine/eGFR normal है, तो pregabalin kidney के लिए generally safe है and is usually the safer/more suitable medicine for diabetic neuropathic pain than tramadol.

For painful diabetic neuropathy, pregabalin is generally the more appropriate and safer long-term option than tramadol, if prescribed by a clinician.
  • Kidney safety: Neither drug usually directly damages the kidneys.
  • Pregabalin: It is eliminated through the kidneys, so it does not usually harm the kidneys, but the dose must be reduced if kidney function declines. With normal creatinine and normal eGFR/creatinine clearance, it is commonly used safely.
  • Tramadol: It and its active metabolite also depend partly on kidney clearance. It is usually not preferred for long-term diabetic nerve pain because it can cause drowsiness, constipation, nausea, falls, dependence, seizures, and drug interactions.
Bottom line: With normal kidney function, pregabalin is usually preferred for diabetic neuropathic pain. It does not generally injure the kidneys, but kidney function should be checked periodically and the dose adjusted if eGFR falls.
Do not take pregabalin and tramadol together unless the prescriber specifically advises it. The combination can increase severe sleepiness, dizziness, falls, and rarely slow breathing.
