Indomethacin in Polyhydramnios

Mechanism

Indomethacin is a COX (cyclooxygenase) inhibitor (NSAID). It reduces amniotic fluid volume by:
  • Inhibiting prostaglandin-mediated suppression of antidiuretic hormone, thereby reducing fetal urine output
  • Exerting direct effects on fetal renal blood flow
These effects reduce fetal urine production, which is the primary source of amniotic fluid in the second half of pregnancy.

Dosage

ParameterRegimen
Loading dose50 mg orally
Maintenance dose25-50 mg orally every 6 hours
DurationGenerally limited to 48 hours
Gestational age limitBefore 32 weeks gestation
"The usual regimen is a 50-mg oral loading dose followed by 25 to 50 mg by mouth every 6 hours. Therapy is generally limited to 48 hours and to pregnancies before 32 weeks' gestation."

Key Fetal Side Effects

These are the main reasons for the strict gestational age and duration limits:
  1. Ductal constriction - transient constriction of the ductus arteriosus occurs in up to 50% of fetuses treated before 32 weeks; usually resolves within 24 hours of stopping the drug. Persistent use can cause irreversible right-sided heart failure.
  2. Oligohydramnios - paradoxically, over-reduction of amniotic fluid can occur with prolonged use (the desired effect becomes an adverse effect).
  3. Neonatal pulmonary hypertension - associated with fetal NSAID exposure, particularly when used near delivery.
  4. NEC, small bowel perforation, IVH - reported with uncontrolled use beyond standard protocols.

Monitoring Requirements

Before and during treatment:
  • Assess amniotic fluid volume and fetal renal anatomy before starting
  • If treatment extends beyond 48 hours (exceptional circumstances only): monitor amniotic fluid volume and ductal blood flow by echocardiography

Contraindications to Use

  • Fetal renal anomalies
  • Pre-existing oligohydramnios
  • Ductal-dependent cardiac defects
  • Twin-twin transfusion syndrome
  • Gestational age ≥ 32 weeks

Clinical Note

When polyhydramnios is associated with preterm labor, indomethacin serves a dual purpose - tocolysis AND reduction of amniotic fluid volume. Repeated courses should be avoided where possible, and treatment should be stopped if delivery is imminent.
