levothyroxine missed dose take two doses next day patient guidance

For one missed levothyroxine (thyroxine) tablet, guidance varies:
  • Take it when you remember if it is not close to the next scheduled dose.
  • If it is nearly time for the next one, skip the missed tablet and take your normal dose.
  • Do not take two tablets together unless your doctor or pharmacist has specifically told you to.
Some thyroid specialists say a single missed daily dose can be made up as two doses the next day because levothyroxine stays in the body a long time. However, NHS guidance advises not taking two doses at once, so the safer general approach is to skip it if you are unsure. The NHS missed-dose advice supports this approach; the American Thyroid Association leaflet notes that one missed dose may be doubled the following day.
Do not double up if you missed more than one dose, or if you have heart disease, palpitations/irregular heartbeat, or are pregnant without checking your clinician. Contact your pharmacist or prescribing doctor for advice.
