Fever grades can vary slightly by source and site of measurement. A commonly used clinical classification is:

Low-grade fever37.3-38.0°C (99.1-100.4°F)
Moderate-grade fever38.1-39.0°C (100.6-102.2°F)
High-grade fever39.1-41.0°C (102.4-105.8°F)
Hyperpyrexia>41.0°C (>105.8°F)
A temperature ≥38.0°C (100.4°F) is generally considered fever, depending on how and where it was measured. Temperatures differ by route: rectal readings are usually higher than oral, while axillary readings tend to be lower. NCBI fever reference
Urgent assessment is appropriate for temperature near or above 40°C, or for fever with confusion, breathing difficulty, seizure, stiff neck, severe headache, rash, dehydration, or in an infant younger than 3 months.
