Pediatric Population (under 12 years)

Important regulatory update: Following a 2014 European review of cardiac risk, domperidone efficacy in children under 12 has not been established. The 1 mg/mL "paediatric" suspension (Motilium) was withdrawn from the market (effective September 2019 in several EU countries) due to lack of proven efficacy and cardiac safety concerns.
If used off-label in children (under medical supervision), the historical dose that was reviewed was:
  • 0.25 mg/kg/dose, up to 3 times daily (maximum)
  • This was reduced from a previous 0.25-0.5 mg/kg dose range specifically to lower cardiotoxicity risk

Key Safety Points

  • Risk of serious ventricular arrhythmias / sudden cardiac death - higher risk in: age >60, doses >30 mg/day, concurrent QT-prolonging drugs or CYP3A4 inhibitors
  • Contraindicated in QTc prolongation, electrolyte disturbances (hypokalemia, hyperkalemia, hypomagnesemia), bradycardia, congestive heart failure
  • Contraindicated with strong CYP3A4 inhibitors (e.g., ketoconazole, fluconazole, erythromycin, clarithromycin)
  • Use at the lowest effective dose for the shortest duration
