2010 ACR/EULAR Classification Criteria for Rheumatoid Arthritis

Step 1 - Entry Requirements (Both Must Be Met)

Before applying the scoring system, the following must be confirmed:
  1. At least 1 joint with definite clinical synovitis (swelling)
  2. The synovitis is not better explained by another disease
If the patient has typical RA erosions on x-ray, they can be classified as RA directly - no scoring needed.

A. Joint Involvement (0-5 points)

Key Points

  • ACPA (anti-CCP) has ~93-98% specificity for RA - even in the first few weeks of illness, its presence is strongly suggestive of aggressive erosive disease
  • ~75% of RA patients are RF positive; ~70% are ACPA positive; ~15% are seronegative for both
  • The 2010 criteria replaced the 1987 ACR criteria - the older criteria required morning stiffness ≥1 hour, rheumatoid nodules, and symmetric arthritis, but were poor at identifying early disease
  • The 2010 criteria are weighted toward early diagnosis - they do not require 6 weeks of disease duration (duration <6 weeks still scores 0, but you can still reach ≥6 total)
  • Patients who cannot be classified by scoring but develop typical erosive disease over time can be reclassified retrospectively
