Fatty liver is often asymptomatic. When signs are present, the cardinal clinical finding is:
  • Hepatomegaly: enlarged, smooth, usually non-tender liver
  • Mild right upper abdominal discomfort or fullness
  • Fatigue or malaise
  • In more advanced disease: features of chronic liver disease, such as jaundice, ascites, pedal edema, spider angiomas, or splenomegaly
On ultrasound, typical signs are a bright echogenic liver, increased liver-kidney contrast, and poor visualization of deep hepatic vessels/diaphragm.
