ACC AHA ECG ST segment elevation interpretation acute myocardial infarction criteria

ECG interpretation from this photographed tracing:
  • Rhythm: Sinus rhythm
  • Rate: about 68 bpm
  • Axis: Normal QRS axis, approximately +38°
  • Intervals: PR about 164 ms; QTc about 401 ms, both within normal limits
  • QRS duration: about 110 ms, borderline prolonged but not a definite complete bundle-branch block
  • Precordial leads: Delayed R-wave transition / poor R-wave progression across V1-V4, with dominant S waves persisting into the anterior chest leads.
  • ST-T changes: I do not see convincing acute, diagnostic ST-segment elevation, significant reciprocal ST depression, or dangerous T-wave changes in this photo.
Impression: Sinus rhythm with normal axis and borderline intraventricular conduction delay, plus poor R-wave progression/delayed precordial transition. No clear acute STEMI pattern visible.
Poor R-wave progression can result from chest-lead placement, normal anatomical variation, prior anterior infarction, ventricular hypertrophy, or conduction abnormalities. It cannot be diagnosed confidently from a photographed ECG alone, so a repeat correctly placed 12-lead ECG, comparison with prior ECGs, and clinical correlation are appropriate.
If this ECG was done for new chest pain, breathlessness, sweating, fainting, palpitations, or pain radiating to the arm/jaw, seek emergency assessment now. A single ECG can miss evolving acute coronary syndrome, and serial ECGs plus troponin testing may be needed, as noted in emergency-medicine guidance.

  • Rhythm: Sinus rhythm with mild sinus arrhythmia
  • Rate: ~74 bpm
  • Axis: Normal, about +43°
  • Intervals: PR 136 ms, QRS 88 ms - normal
  • QT interval: QT 468 ms, machine-calculated QTc ~519 ms, which is prolonged
  • Voltage: Borderline/minimal voltage criteria for LVH, which may be a normal variant. ECG voltage alone does not diagnose LVH.
  • ST-T segments: Mild nonspecific ST-T changes. I do not see a clear acute ST-elevation myocardial infarction pattern in this photograph.
Main finding: prolonged QTc. The automated QTc can be inaccurate and should be manually checked on a clean repeat ECG, but a QTc around 519 ms, if confirmed, deserves prompt clinical review because QT prolongation can predispose to torsades de pointes, a dangerous ventricular arrhythmia. QT prolongation may be caused by medicines, low potassium/magnesium/calcium, bradycardia, medical illness, or less commonly inherited long-QT syndrome. The clinical long-QT review advises diagnosis only after QTc prolongation is confirmed on repeated 12-lead ECGs.
Recommended next steps: Arrange same-day clinician review for repeat ECG with manual QT/QTc measurement, medication review, and serum potassium, magnesium, calcium, and renal function. Do not stop prescribed medication without medical advice, but inform the clinician about all drugs, including antibiotics, antiemetics such as ondansetron, antidepressants/antipsychotics, antihistamines, and any supplements.
Emergency assessment now if there is fainting, seizure-like episode, sustained palpitations, dizziness, chest pain, or breathlessness.
