Extra-Articular Manifestations of Rheumatoid Arthritis (RA)
1. Skin / Subcutaneous
Rheumatoid Nodules (most common - 20% of patients)
- Firm, semi-mobile subcutaneous nodules
- Located over extensor surfaces and pressure-bearing areas (olecranon, sacrum, Achilles tendon)
- Size: few mm to 5 cm
- Usually asymptomatic; can ulcerate
- Only in seropositive patients (moderate-to-high RF titers)
- Accelerated nodulosis - sudden multiple nodules after starting methotrexate
Rheumatoid Vasculitis (rare - 2-5%)
- Small vessel: palpable purpura, nail fold infarcts
- Medium vessel: nodules, ulcers, digital gangrene, necrotizing livedo reticularis
- Systemic vasculitis: mononeuritis multiplex, mesenteric infarction, renal infarction
- Occurs with high-titer RF + erosive arthritis + nodules
Other Skin Features
- Pyoderma gangrenosum
- Rheumatoid neutrophilic dermatitis
- Palisaded neutrophilic and granulomatous dermatitis (PNGD)
- Fragile skin (also from corticosteroid use)
2. Pulmonary (common, often subclinical)

- Scleritis - painful, serious; can lead to scleromalacia perforans (blue sclera)
- Corneal melt (peripheral ulcerative keratitis)
5. Neurological
- Mononeuritis multiplex - from vasculitis of vasa nervorum
- Peripheral neuropathy - sensorimotor
- Cervical myelopathy - from atlantoaxial subluxation (C1-C2 instability due to erosion of transverse ligament/odontoid)
- Exam point: Even asymptomatic RA patients may have severe cervical stenosis on MRI
- High risk during intubation - requires manual in-line stabilization
- Carpal tunnel syndrome (median nerve compression)
6. Haematological
- Anaemia of chronic disease (most common systemic finding overall)
- Felty's syndrome - triad of: RA + splenomegaly + neutropenia
- Risk of infections and leg ulcers
- Thrombocytosis (reactive, correlates with disease activity)
- Large granular lymphocyte (LGL) syndrome
7. Renal
- Amyloidosis (AA type) - secondary amyloid from chronic inflammation; presents as proteinuria/nephrotic syndrome
- Glomerulonephritis
- Tubulointerstitial nephritis
- Drug-related nephrotoxicity (NSAIDs, gold, penicillamine)
8. Hepatic
- Elevated liver enzymes (common, often drug-related)
- Hepatic fibrosis
- Felty's syndrome can cause portal hypertension
9. Lymphoid / Malignancy
- Increased risk of Non-Hodgkin's lymphoma (especially with high disease activity)
- Malignancy is one of the three extra-articular features causing premature death
10. Sjogren Overlap
- Sicca syndrome (dry eyes + dry mouth)
- Secondary Sjogren syndrome seen in RA
