mMRC (Modified Medical Research Council) Dyspnea Scale

The mMRC is a 5-point self-assessment scale (Grades 0-4) that quantifies breathlessness based on activity level. Higher scores indicate greater impairment.

Grading

0Not troubled by breathlessness except with strenuous exercise
1Short of breath when hurrying on level ground or walking up a slight hill
2Walks slower than people of the same age on level ground due to breathlessness, OR has to stop to catch breath when walking at own pace on level ground
3Stops for breath after walking ~100 yards (91 m) or after a few minutes on level ground
4Too breathless to leave the house, OR breathless when dressing/undressing

Key Clinical Uses

GOLD ABCD Classification (COPD): An mMRC score ≥ 2 places a patient in the "high symptom" category (Groups B or D).

A BODE score > 7 carries ~30% 2-year mortality; score < 5 carries < 10% 2-year mortality.

Notes

  • The cutoff mMRC ≥ 2 is the threshold used in GOLD guidelines to define "more symptomatic" patients requiring more aggressive bronchodilator therapy.
  • The mMRC focuses purely on activity-related breathlessness and does not capture cough, sputum, or sleep disturbance - for those, the CAT (COPD Assessment Test) score ≥ 10 is the equivalent "high symptom" threshold.
  • The scale was originally developed by the British Medical Research Council and later modified (hence "m"). The MRC itself notes it cannot grant permission to use modified versions, though the mMRC is freely used in research and guidelines.
