Terbinafine resistance in Trichophyton species - specifically T. rubrum, T. interdigitale, T. mentagrophytes, aur newly emerging T. indotineae - SQLE gene mein point mutations ke karan hoti hai.
Mechanism
- Terbinafine = squalene epoxidase enzyme ka inhibitor hai
- Ye enzyme ergosterol biosynthesis pathway mein kaam karta hai
- SQLE gene mein mutation hone se enzyme ki structure change hoti hai → terbinafine binding kam hoti hai → resistance

Clinical Context
- T. indotineae India mein epidemic level par hai - terbinafine se treatment fail hoti hai
- In cases mein itraconazole (higher dose, longer duration) use hota hai
- Both terbinafine + itraconazole fail ho toh: voriconazole / posaconazole (off-label)
