OSCE History Checklist: Erectile Dysfunction (ED)

1. Preparation and opening

  • Wash hands, introduce yourself, confirm the patient's name and date of birth
  • Explain the purpose and ask for consent to discuss sensitive topics
  • Ensure privacy and use clear, non-judgmental language
  • Ask an open question: "What has brought you in today?"
  • Ask whether he would like a chaperone or partner involved

2. Presenting complaint

Textbooks define ED as the inability to attain or maintain an erection sufficient for satisfactory sexual performance (NIH Consensus, quoted in Textbook of Family Medicine 9e, p. 1193).
  • Onset: sudden or gradual? When did it start?
  • Duration and course: constant, worsening or intermittent?
  • Situational or global: Does it happen with a partner only, or also during masturbation?
  • Morning or nocturnal erections: present or absent? Spontaneous erections suggest a psychogenic cause. Absent erections suggest an organic cause.
  • Type of difficulty: failure to get an erection, failure to maintain it, or reduced rigidity?
  • Triggers: new partner, stress, or a specific event such as surgery, trauma or starting a new drug
  • Erection quality: Ask him to score it out of 10 and say whether penetration is possible.
  • Previous treatments: PDE-5 inhibitors, herbal or online products, and whether they worked
Campbell-Walsh and Smith and Tanagho's General Urology both stress that dysfunction may be situational. A patient may masturbate and wake with morning erections but fail during partnered intimacy.

3. Screen other sexual domains

  • Libido: reduced desire suggests hypogonadism, depression or a drug effect
  • Ejaculation: premature, delayed, absent, or reduced volume
  • Orgasm: anorgasmia
  • Penile curvature, pain or plaques: Peyronie's disease
  • Priapism history
  • Validated tool: offer the Sexual Health Inventory for Men (SHIM / IIEF-5) to quantify severity

4. Psychosocial and relationship history

  • Mood: depression, anxiety, performance anxiety
  • Stressors: work, finances, bereavement
  • Relationship: quality, new partner, conflict, partner's sexual health or menopause, and the partner's attitude
  • Past sexual trauma or abuse
  • Sexual orientation and practices, asked sensitively
  • Expectations and beliefs, including cultural or religious factors
  • Pornography use
  • Safe sex and STI risk

5. Screen for organic causes (by system)

Causes are vasculogenic, psychological, endocrine, neurologic, iatrogenic, drug-related, systemic illness and ageing (Goldman-Cecil Medicine).
Vascular and cardiac
  • Hypertension, hyperlipidaemia, coronary heart disease, peripheral vascular disease, stroke
  • Exertional chest pain, breathlessness and exercise tolerance. This matters because he may need a cardiac fitness check before sexual activity or a PDE-5 inhibitor.
Endocrine and metabolic
  • Diabetes: duration, control, complications
  • Hypogonadism symptoms: low energy, reduced muscle bulk, reduced body hair, gynaecomastia, hot flushes
  • Thyroid disease
  • Hyperprolactinaemia: galactorrhoea, headache, visual field loss
  • Obesity and metabolic syndrome
Neurological
  • Multiple sclerosis, Parkinson's disease, stroke, spinal cord injury, neuropathy, epilepsy
  • Sensory loss in the saddle area, bladder or bowel symptoms (consider cauda equina or a spinal cause)
Urological and pelvic
  • Lower urinary tract symptoms or BPH
  • Prostate cancer or prostate surgery (radical prostatectomy)
  • Pelvic or perineal trauma, radiotherapy, or cycling
  • Peyronie's disease, previous penile surgery, hypospadias
  • Testicular problems or an undescended testis
Other systemic disease
  • Chronic kidney disease, liver disease (cirrhosis), COPD, HIV
  • Obstructive sleep apnoea. Ask about snoring, witnessed apnoeas and daytime sleepiness (listed as a symptom in Murray and Nadel's Respiratory Medicine).
Mental health
  • Depression, anxiety, psychosis, eating disorders

6. Medication history

Drugs contribute in up to 25% of cases (Textbook of Family Medicine 9e).
  • Antihypertensives: thiazides, beta-blockers, spironolactone
  • Antidepressants: SSRIs, TCAs
  • Antipsychotics
  • Anti-androgens and 5-alpha reductase inhibitors: finasteride, GnRH analogues
  • Other drugs: opioids, benzodiazepines, antiepileptics, H2 blockers
  • Nitrates or nicorandil: these are contraindications to PDE-5 inhibitors, so always ask
  • Current PDE-5 inhibitor use: sildenafil, tadalafil
  • Allergies
  • Over-the-counter, herbal and online medications, and anabolic steroids

7. Lifestyle and risk factors

  • Smoking: pack-years
  • Alcohol: units per week
  • Recreational drugs: cannabis, cocaine, opioids, amphetamines
  • Exercise and diet, and weight change
  • Sleep
  • Occupation: for example, prolonged cycling or vibrating machinery

8. Past medical, surgical, family and social history

  • Previous pelvic, prostate, colorectal, vascular or spinal surgery
  • Family history of early cardiovascular disease and diabetes
  • Living situation, occupation and support

9. Red flags and risk stratification

  • Cardiovascular risk: ED can precede cardiac symptoms and acts as a sentinel for future cardiovascular events. The Prostate Cancer Prevention Trial found about a 45% higher risk of cardiac events at 5 years (Campbell-Walsh Urology, p. 2010 to 2011).
  • Suspected cauda equina: saddle numbness, bladder or bowel dysfunction
  • Hypogonadism
  • Suicidal ideation if depression is suspected

10. Ideas, concerns and expectations (ICE)

  • What does he think is causing it?
  • What worries him most (for example, cancer, heart disease, relationship breakdown)?
  • What does he hope for from the consultation?

11. Closing

  • Summarise back to the patient and check you have understood correctly
  • Ask "Is there anything else you would like to mention?"
  • Outline next steps:
    • Examination: genital, secondary sex characteristics, BP, BMI, peripheral pulses and neurological examination
    • Investigations: HbA1c or fasting glucose, lipids, early-morning testosterone, and renal and liver function. Prolactin, LH and free testosterone are reserved for suspected hypogonadism (Textbook of Family Medicine 9e).
    • Management: lifestyle changes, medication review, PDE-5 inhibitors, and psychosexual counselling
  • Thank the patient and offer written information

Quick mnemonic

"ERECTILE"
  • Erection quality (onset, course, morning erections)
  • Relationship and psychological factors
  • Ejaculation and libido
  • Cardiovascular and other systemic disease
  • Treatments and medications (nitrates!)
  • Injury, surgery, neurological disease
  • Lifestyle (smoking, alcohol, drugs)
  • Endocrine (diabetes, hypogonadism)

Recent evidence flag

A 2026 systematic review and meta-analysis (PMID 41319020) reports de novo ED after a first myocardial infarction. This supports asking about cardiac history and exertional symptoms. It does not contradict the textbook content above.
I didn't fetch the guideline-specific wording from the AUA or EAU. If your examiners use a particular marking scheme, check it against your local guidance.

ED History OSCE Revision Card

PDF Document · PDF

I rendered it to check the layout. It fits on a single A4 page with a tick-box next to every item.
What's on the card:
  • Eleven sections in two columns: opening, presenting complaint, other sexual domains, psychosocial, organic causes by system, drug history, lifestyle, PMH/FH/SH, red flags, ICE, and closing with next steps.
  • Highlighted points: the nitrate/nicorandil contraindication to PDE-5 inhibitors is in red. The point that ED is a sentinel for cardiovascular disease is in bold.
  • ERECTILE mnemonic strip along the bottom as a last-minute prompt.
  • Footer: the textbook sources and a reminder to check your local OSCE mark scheme and current AUA/EAU guidance.
