Can I know the pathology of all the teeth present in this image

Here is a detailed radiographic (OPG) analysis of all teeth visible in this panoramic X-ray, using FDI notation:

Panoramic Radiograph (OPG) Analysis

Important disclaimer: This analysis is for educational/informational purposes. A licensed dentist or oral radiologist must review the original high-resolution film for definitive clinical diagnosis and treatment planning.

Artifact Note

A metallic radiopaque object (nose ring/septum piercing) is visible in the midline above the maxillary anterior teeth. This was not removed before the X-ray and creates a superimposition artifact over the nasal region.

Quadrant 1 - Upper Right (Teeth 11-18)

ToothFinding
18Absent - either congenitally missing (agenesis) or previously extracted
17Present - no obvious caries, periapical pathology, or restorations visible
16Present - normal radiographic appearance
15Present - normal radiographic appearance
14Present - normal radiographic appearance
13Present - normal radiographic appearance
12Present - normal radiographic appearance
11Present - normal radiographic appearance

Quadrant 2 - Upper Left (Teeth 21-28)

ToothFinding
21Present - normal radiographic appearance
22Present - normal radiographic appearance
23Present - normal radiographic appearance
24Present - normal radiographic appearance
25Present - normal radiographic appearance
26Present - normal radiographic appearance
27Present - normal radiographic appearance
28Absent - either congenitally missing or previously extracted

Quadrant 3 - Lower Left (Teeth 31-38)

ToothFinding
31Present - normal radiographic appearance
32Present - normal radiographic appearance
33Present - normal radiographic appearance
34Present - normal radiographic appearance
35Present - normal radiographic appearance
36Present - normal radiographic appearance
37Present - normal radiographic appearance
38Mesioangularly impacted - the tooth is tilted forward and wedged against the distal aspect of tooth 37. Roots are fully formed. A slight radiolucency around the crown may indicate a follicular (dentigerous) cyst or pericoronitis - warrants further evaluation and likely surgical extraction

Quadrant 4 - Lower Right (Teeth 41-48)

ToothFinding
41Present - normal radiographic appearance
42Present - normal radiographic appearance
43Present - normal radiographic appearance
44Present - normal radiographic appearance
45Present - normal radiographic appearance
46Present - normal radiographic appearance
47Present - normal radiographic appearance
48Mesioangularly impacted - similar pattern to tooth 38, leaning against the distal aspect of tooth 47. Fully formed roots. Risk of caries on the distal of 47 due to impaction position

Summary of Key Findings

FindingTeeth Involved
Bilateral impacted lower wisdom teeth38, 48 (mesioangular impaction)
Missing upper wisdom teeth18, 28 (absent)
No obvious caries/restorationsOverall dentition appears caries-free on this view
No periapical pathologyNo radiolucencies at root apices
No visible root canal treatments or crownsNone identifiable
Alveolar bone levelsWithin normal limits; no significant horizontal or vertical bone loss
TMJ condylesAppear symmetric and rounded
Metallic artifactNose piercing superimposed over anterior maxillary region

Clinical Recommendations

  1. Teeth 38 and 48 - Referral to oral surgeon for assessment and likely surgical extraction due to mesioangular impaction. The slight follicular space around 38 needs monitoring for cyst formation.
  2. Bitewing radiographs should be taken separately to rule out interproximal (between-teeth) caries, which are not reliably detected on an OPG alone.
  3. Missing 18 and 28 - confirm whether congenital or post-extraction via patient history.
  4. The overall dental health appears good with a full adult dentition (except absent upper third molars and impacted lower third molars).
