Seizure mein Metoclopramide kyun dete hain?
Kab diya jaata hai:
Seizure ke baad (postictal phase) mein patient ko aksar nausea aur vomiting hoti hai. Ye antiemetic ke tor par diya jaata hai taaki:
- Patient ko vomiting se aspiration na ho (aspiration pneumonia ka risk)
- Oral/IV medications dene mein aasani ho
- Patient comfortable rahe
Metoclopramide ka mechanism:
- Dopamine D2 receptor antagonist hai - CTZ (Chemoreceptor Trigger Zone) mein dopamine ko block karta hai, isse nausea/vomiting rukti hai
- Saath mein prokinetic bhi hai - gastric emptying tez karta hai
Lekin ek IMPORTANT problem hai:
Metoclopramide khud seizure threshold NEECHE kar sakta hai - yaani seizures ka risk badh jaata hai, especially in patients with:
- End-stage kidney disease (ESKD) - jahan drug accumulate ho
- Already epileptic patients
Yeh Brenner & Rector's Kidney textbook mein clearly likha hai: "A number of drugs may lower the seizure threshold... including metoclopramide."
Kya Ondansetron (Ondasetron) de sakte hain?
Haan - Ondansetron BETTER choice hai seizure patients mein!
Tintinalli's Emergency Medicine clearly kehta hai:
"Ondansetron is the preferred antiemetic to control associated nausea or vomiting. Do not use phenothiazines (i.e., promethazine) because these agents lower the seizure threshold."
Ondansetron ka mechanism:
- 5-HT3 (Serotonin) receptor antagonist hai
- CTZ aur GI tract mein serotonin receptors block karta hai
- Seizure threshold affect NAHI karta - isliye safer hai
