For most adults with type 2 diabetes already taking metformin, dapagliflozin is added rather than replacing metformin:
  • Continue metformin at the current tolerated dose, usually taken with meals.
  • Add dapagliflozin 5 mg once daily, usually in the morning, with or without food.
  • Reassess glucose/HbA1c and tolerance after about 8-12 weeks. If more glucose lowering is needed, increase dapagliflozin to 10 mg once daily.
Example: if someone takes metformin 1,000 mg twice daily and tolerates it, a common regimen is to continue that and add dapagliflozin 5 mg each morning, later increasing dapagliflozin to 10 mg each morning if appropriate.
